IHCRC uses a patient-centered medical home model of care. The patient-centered medical home is a team approach to health care. Your medical home team will include your health care provider, others who support you, and you. In the patient-centered medical home, you are the most important person on the healthcare team. Caring staff and state-of-the-art equipment make certain you receive the best healthcare available. Wait times are shorter. You see the same doctor and medical team on every visit. Most everything you need for your general health is available right here.
Primary care (general medicine, “sick” care, and wellness care)
Internal medicine (chronic conditions like diabetes, arthritis, and high blood pressure)
Pediatrics and well-baby care
Public health (testing and treatment for sexually transmitted diseases, immunizations, and prenatal/newborn education)
Medication adherence and therapy management
Women’s health (prenatal care, family planning, breast health,

To request a CD with images from Radiology please allow 24-48 hours. Radiology will call when the CD is ready for pick up.
Same Day Clinic Visits
Same Day visits are for sick visits only. For routine health concerns, you will need to schedule with your Primacy Care Physician. Ongoing or routine concerns such as diabetes, thyroid disorders, physicals, or referrals are examples of care that should be scheduled.

Closures
The clinic is closed the first Wednesday of each month and on all federal holidays.
