IHCRC Chart
Need to update your address or last name? You can update your chart online or in-person. To update your chart online please select the "Update Adult Chart" button below. The "Acknowledgment of Privacy Practices" form must be updated annually before your first appointment of the year. If you have any questions, please speak to someone at the front desk or call (918) 588-1900 and ask for Patient Registration.

Walk-in Hours
Monday: 6:45 am – 5:00 pm
Tuesday - Friday: 6:45 am – 5:15 pm

Closures
The clinic is closed the first Wednesday of each month and on all federal holidays.
For medical emergencies, dial 911 or go to the nearest emergency room.
