Behavioral Health Services Request
Now accepting patient enrollment paperwork for appointments from 6:45 am - 3:00 pm. Paperwork will not be accepted without an active chart. If you do not have an active chart visit our New Patient page to open a chart.
After completion of the Behavioral Health Services request form, a staff member will contact you to schedule an intake for Behavioral Health Services. This initial appointment will take approximately 2 hours to complete and will determine what services are provided. Arrive 15 minutes before your appointment to complete additional paperwork. Please note that if you miss your appointment, you will be required to fill out a new request for services.
Limited Access
If you are on limited access and are in need of services as a new patient in the Behavioral Health Department, you will need to visit the clinic to fill out paperwork. The process for appointments while on limited access will be explained at that time.

Closures
The clinic is closed the first Wednesday of each month and on all federal holidays.
For medical emergencies, dial 911 or go to the nearest emergency room.
