JOB SUMMARY
We are looking for a compassionate and dedicated Pharmacist to join our team as an Integrated Pharmacist. The Integrated Pharmacist plays a vital role on the interdisciplinary care team, working closely with medical providers, behavioral health professionals, and care coordinators to optimize medication for patients with complex, chronic, or acute conditions. This role goes beyond traditional dispensing to include direct patient care, medication reconciliation, education, and quality improvement initiatives within a collaborative care model.
ESSENTIAL JOB FUNCTIONS
Performs duties within the scope of licensure and professional ethics standards.
Accurately and efficiently dispenses prescribed drugs per provider instructions and established guidelines.
Consults with medical staff about potential drug interactions, patient medical history, and the use of particular medications.
Counsel’s patients about medications and dosing instructions.
Other duties as assigned.
REQUIRED LICENSE, CERTIFICATION, EDUCATION, SPECIALIZED SKILLS
Current and valid Oklahoma PharmD or DPh license from an accredited school of Pharmacy program.
Must be registered with the Oklahoma State Board of Pharmacy.
2+ years working in a clinical environment preferred.
BLS CPR certification.
Intermediate computer skills.
Strong written and communication skills.
Excellent interpersonal skills.
Ability to multi-task.
