JOB SUMMARY
We are seeking a Youth and Family Social Worker to join our growing Behavioral Health Department! The Youth and Family Social Worker screens patients for wellbeing and behavioral health concerns in order to provide direct link to Pediatric Behavioral Health's programs and services. Youth and Family Social Worker works directly with behavioral health and medical providers to offer check-ins and follow-ups with patients between appointments to support treatment.
ESSENTIAL JOB FUNCTIONS
Provides brief interventions when appropriate.
Assists with coordination of grant requirements and data collections.
Assists and supports ongoing clinic activities that provide opportunity for youth to connect to culture and wellness.
Screens Pediatric patients for wellbeing and behavioral health concerns
Links, advocates, refers families to resources.
Contacts patients identified by behavioral health or medical providers as needing support services/case management.
Ability to initiate conversations and consultations with the multidisciplinary team of providers at the clinic.
Facilities group classes with emphasis on youth needs (Parenting, Adult Life Skills)
Complies with all IHCRC policies and procedures.
Able to receive feedback and make changes as necessary.
Makes attempts to communicate and ask questions to supervisor.
Ability to be flexible in day-to-day tasks.
Maintains an organized process to ensure follow up with patients is done consistently and in a timely manner.
Timely and accurate completion of progress notes.
Ability to prioritize needs and manage time effectively.
Actively work towards contributing to the mission of IHCRC.
Maintains a positive and professional attitude.
Ability to work outside of regular clinic hours for events/activities; Some Saturdays required.
Other duties as assigned.
REQUIRED LICENSE, CERTIFICATION, EDUCATION, SPECIALIZED SKILLS
Bachelors of Social Work or Master of Social Work from an accredited program.
Certification as a Licensed Social Work Associate (LSWA). Must keep current and maintain CEU requirement.
2+ years working in social services or mental health field.
Current Oklahoma Drivers License with a clear DMV background check. Must provide proof of vehicle liability insurance.
Intermediate computer skills and proficiency with electronic health records software as appropriate.
Must be a self-starter, multi-tasker, with ability to effectively give direction and input to care team.
