JOB SUMMARY

Join our team in making a meaningful impact to Empower the American Indian through exceptional healthcare! We are seeking a RN to join our organization as the Elder Educator. The Elder Educator plays a key role in promoting health and wellness among our aging patient population by providing education, resources, and compassionate support for all aspects of health and wellbeing. The Elder Educator works collaboratively with patients, caregivers, and interdisciplinary teams to empower seniors to live healthier, more independent lives.

ESSENTIAL JOB FUNCTIONS

  • Provide ongoing education in one on one and group-based settings to the aging population and caregivers.

  • Work with CQI dept. to conduct assessments and surveys to determine the health education needs of IHCRC gerontology patients.

  • Assist patients and caregivers in finding health services or information.

  • Assist case manager with care transition needs of aging patients.

  • Assist medical team with care of elder patients in clinic as needed.

  • Participate in educational opportunities for nurse training programs related to aging population.

  • Participate in distributing health education materials and aids, including pamphlets, notices, videos, audio clips, images, and posters for aging populations and caregivers.

  • Coordinate with other IHCRC departments for special events that promote mental health and medical health education.

  • Evaluate the effectiveness of the programs and educational materials. 

  • Document activities and recording information related to events offered and attended. Work with Administrative Assistant to keep sign-in logs for all activities.

  • Perform other duties as assigned under the Pathways of Community Living grant.    

  • Make home visits to patients residence, as needed, for telehealth support. Collect vital signs, draw labs as needed, and provide education for access to telehealth visits with primary care providers.

  • Provide case manager assistance to all homebound/telehealth gerontology patients.

  • May be required to work some Saturdays for special events with a trade off day during the same work week.

REQUIRED LICENSE, CERTIFICATION, EDUCATION, SPECIALIZED SKILLS

  • Associates Degree in Nursing required. BSN preferred. Current and Valid Oklahoma RN License.

  • BLS CPR Certification.

  • Current Oklahoma Drivers License with a clear DMV background check. Must provide proof of vehicle liability insurance. 

  • 3+ years clinical experience with primary care and/or specific population preferred.

  • Excellent communication and networking skills.

  • Advanced ability to plan, implement, monitor, and evaluate health education programs.

  • Ability to develop educational materials and aids, and to promote and present health education initiatives.
