JOB SUMMARY

We are seeking a compassionate and dedicated RN Elder Case Manager to join our growing clinic! The RN Case Manager provides comprehensive, patient-centered care coordination for elderly patients in a nonprofit outpatient clinic setting. This role focuses on supporting older adults with complex medical, social, and functional needs by coordinating services, promoting independence, and improving overall health outcomes while aligning care with the clinic’s mission to serve vulnerable and underserved populations.

ESSENTIAL JOB FUNCTIONS

Provide ongoing education in one on one and group-based settings to the aging population and caregivers.

Work with CQI dept. to conduct assessments and surveys to determine the health education needs of IHCRC gerontology patients.

Assist patients and caregivers in finding health services or information.

Assist case manager with care transition needs of aging patients.

Assist medical team with care of elder patients in clinic as needed.

Participate in educational opportunities for nurse training programs related to aging population.

Participate in distributing health education materials and aids, including pamphlets, notices, videos, audio clips, images, and posters for aging populations and caregivers.

Coordinate with other IHCRC departments for special events that promote mental health and medical health education.

Evaluate the effectiveness of the programs and educational materials. 

Document activities and recording information related to events offered and attended. Work with Administrative Assistant to keep sign-in logs for all activities.

Perform other duties as assigned under the Pathways of Community Living grant.    

Make home visits to patients residence, as needed, for telehealth support. Collect vital signs, draw labs as needed, and provide education for access to telehealth visits with primary care providers.

Provide case manager assistance to all home bound/telehealth gerontology patients.

May be required to work some Saturdays for special events with a trade off day during the same work week

REQUIRED LICENSE, CERTIFICATION, EDUCATION, SPECIALIZED SKILLS

Associates Degree in Nursing required. BSN preferred. Current and Valid Oklahoma RN License.

BLS CPR Certification.

3+ years clinical experience with primary care and/or specific population preferred.

Excellent communication and networking skills.

Advanced ability to plan, implement, monitor, and evaluate health education programs.

Ability to develop educational materials and aids, and to promote and present health education initiatives.

 

WORKING CONDITIONS

MEDICAL:   The employee is on feet most of day. Intermittent physical activity including walking, standing, sitting, lifting and supporting patients is essential. This position performs tasks that involve exposure to patients as well as blood, body fluids, or tissues. Incumbent will be exposed to virus, disease and infection from patients and specimens in working environment. Personal protective equipment use required. Engages in Health Promotion and Disease Prevention.
