"Heart Savers" Diabetes Intervention Program

In 2005, Indian Health Care Resource Center began our "Heart Savers" five-year Cardiovascular Disease Risk Reduction Demonstration Project for adult diabetics. Funded by a grant from the Indian Health Service, the project features an innovative partnership of IHCRC's urban demonstration health center, the Northeastern Tribal Health System (NTHS) Miami Service Unit and the Indian Health Service Model Diabetes Program in Claremore.

The Heart Savers cardiovascular intervention and diabetes management activities helps American Indians with diabetes adopt healthier lifestyles and maintain appropriate weight. Both IHCRC and NTHS are using an integrated curriculum to address a range of health conditions including diabetes, metabolic syndrome, cardiovascular disease, obesity, nutrition, mental health and smoking. A team of qualified health professionals use an interdisciplinary approach to care and case management to provide a comprehensive program of medical, behavioral, nutritional and physical activity components. The Claremore Model Diabetes Prevention Program and the University of Oklahoma College of Medicine-Tulsa provides technical guidance in support of the collaborative project.

Stacy Berg, our Heart Savers Case Manager

Stacy Berg is the case manager for Indian Health Care Resource Center's Heart Savers cardiovascular diabetes program. Stacy was born and raised in Southern California and after graduating from college, moved to Portland and Seattle for several years. She eventually moved to Texas, and in 1995 she arrived in Tulsa. Stacy stays busy outside of work keeping up with her two teenage boys. In 1998, after being a stay-at-home mom for ten years, Stacy decided to return to school at OSU-Tulsa to work on a Master's degree in counseling. She is motivated to assist our diabetic patients reach their program and personal health goals. Stacy meets with patients and work together with them to develop lifelong healthy habits.
