Attendees to this year's Dance of the Two Moons scheduled for April 5 will have the opportunity to see and experience the 2009 Nissan Murano S, with Intelligent CVT (continuously Variable Transmission) 265-hp, 3.5liter DOHC 24-valve V6 engine. Or at the winner's request – $20,000 in lieu of the car.

Only 2,500 will be sold, this vehicle will be raffled off on July 1, 2008 to one lucky ticket holder. For more information see Terms and Eligibility below.

Presented by Jackie Cooper Imports, benefiting Indian Health Care Resource Center of Tulsa, Inc. The proceeds from this raffle will ensure that IHCRC can continue to provide quality health care and valuable camps to area Native American children. These camps provide a place for Native children to learn the importance of well-balanced nutrition, exercise, teamwork, self-esteem, diabetes education and prevention, cultural awareness and communications and problem solving skills.

How to Enter: Complete the raffle ticket request form, indicating the number of raffle tickets requested, and mail it, along with your payment* of $25.00 for each raffle ticket, to: Indian Health Care Resource Center of Tulsa, Inc. (IHCRC), 550 S. Peoria Ave., Tulsa, OK 74120. You may also enter at www.ihcrc.org. For ticket request forms (along with payment in full) received by the IHCRC office on or before June 15, 2008, raffle ticket receipts will be mailed to the address on the response form. After that date, you may pick up your raffle ticket(s) and receipts at the IHCRC office. Raffle tickets may be purchased up until June 30, 2008 (subject to availability). IHCRC reserves the right to extend the sale of tickets by 14 days. No mechanically reproduced, illegible, incomplete, forged or altered response form or tickets will be accepted. IHCRC is not responsible for late, lost, damaged or misdirected postage due to mail.

Only 2,500 tickets will be available for purchase. No more than 100 tickets may be purchased by any individual. Raffle ticket requests will be honored on a first come-first served basis.

Winner: Prize winner will be determined from all eligible raffle tickets purchased in a random drawing to be conducted on or before July 1, 2008 by IHCRC. Odds of winning: 1 in 2,500. Ticket holder need not be present at drawing to win. Winner will be notified by telephone and/or mail if not in attendance at the drawing. Winner must accept prize within 60 days, and provide social security number, and will be required to sign an affidavit of eligibility and release. Prize will be awarded to alternate winner if affidavit is not returned within 7 days after hand delivery or mailing to winner, or if prize notification letter is returned as undeliverable. Winner agrees to use of his/her name, address, likeness and/or prize information for promotional purposes in any medium without compensation, to the extent permitted by law.

Grand Prize: One (l) 2009 Nissan Murano S with Intelligent CVT (continuously Variable Transmission) 265-hp, 3.5liter DOHC 24-valve V6 engine (approximate retail value: $27,700). All dealer prep and destination fees included. If the car is accepted by Grand Prize winner, he/she must present evidence of insurance and current driver's license at time of delivery, and must take delivery at Jackie Cooper Nissan, located at 9898 S. Memorial Drive, Tulsa, OK. Title, registration, insurance, taxes and all other fees not specified herein are the sole responsibility of the winner.

-OR- Upon request of Grand Prize winner, $20,000.00 in cash in lieu of car.

Income Tax Withholding and Reporting to the IRS and Oklahoma Tax Commission: The Internal Revenue Code and the Regulations thereunder, and the Oklahoma Income Tax Code, require Indian Health Care Resource Center to withhold income taxes on the raffle winnings, to pay these withholding taxes to the IRS, and the OTC, and to report the raffle winnings and the withholding taxes to the IRS, OTC, and the winner. The IRS withholding tax rate is either 25% of the value of the automobile, or 25% of the cash prize, and the OTC withholding tax rate is 4%.

If the Grand Prize winner elects, in writing, to receive the automobile, he/she must deliver to Indian Health Care Resource Center two cashier’s or bank certified checks, payable to Indian Health Care Resource Center of Tulsa. One check shall be for $6,925.00, which is 25% of the $27,700.00 value of the automobile. The other check shall be for $1,108.00, which is 4% of the $27,700.00 value.

If the Grand Prize winner elects, in writing, to receive the $20,000.00 cash prize in lieu of the automobile, he/she will receive a check from Indian Health Care Resource Center in the amount of $14,200.00. This check is the cash prize of $20,000.00, reduced by U.S. income tax withholding of $5,000.00, which is 25% of $20,000.00, and Oklahoma income tax withholding of $800.00, which is 4% of $20,000.00.

In either case, Indian Health Care Resource Center will prepare and issue an IRS Form W-2G to the winner, the State of residence of the winner, and the IRS, and will remit the appropriate withholding taxes to the IRS and the OTC. Before delivery of the car or the cash prize, the winner will be required to provide his/her name, address and social security number to Indian Health Care Resource Center, and sign the Form W-2G.

Indian Health Care Resource Center reserves the right to substitute a prize of equal or greater value. No other substitution or transfer of a prize is permitted. Indian Health Care Resource Center is only responsible for the delivery of the prize; it is not responsible for the prize utility, quality, or otherwise.

Eligibility: Raffle is open to any individual legal resident of the United States (excluding territories and possessions) 21 years or older. Employees of IHCRC, Jackie Cooper and their respective directors, trustees, officers and agents, and members of each of their immediate families (includes head of household, spouse (if married) and children under the age of 21) and households (includes any person, regardless of age and regardless of family relationship, who lives in the same household) are not eligible to enter. This offer is void where prohibited or restricted by law and is subject to applicable federal, state and local regulations. Taxes and fees, including State and Federal income taxes, are the sole responsibility of the prize winner.

Winner List: The name of the winner will be available at the IHCRC office in Tulsa at 550 S. Peoria Ave. and can be viewed online at www.ihcrc.org after July 9, 2008.

Miscellaneous: By participating in this raffle, each entrant accepts the conditions stated in these official rules, agrees to be bound by the decisions of the Indian Health Care Resource Center of Tulsa, Inc. Board of Trustees, and warrants that he/she is eligible to participate in this raffle. By accepting prize, winner agrees to release Indian Health Care Resource Center of Tulsa, Jackie Cooper and their receptive directors, trustees, employees, officer, agents, sponsors, advisors, and vendors including without limitation, their advertising and promotion agencies, from any and all liability, loss or damages arising from or in connection with the awarding, receipt, and/or use or misuse of their prize or participation in any prize-related activities. Ticket cost is not tax deductible. In the case of any dispute, IHCRC’s decision shall be final.
